I generally don't italicize punctuation, but I think it's entirely a matter of choice rather than there being a strict stylistic rule.

I can't really think of a situation where punctuation within wouldn't also be italicized, but I suppose there could be one.

Anything that isn't part of the italicized segment isn't italicized. So things like [sic] shouldn't be italicized.

One thing I CAN say about women, or at least, the media-created image of women and the way they interact with men (this is all I actually know of the world) is that they do, in fact, claim all nice guys are gay when in actuality they are so spoiled that when guys are nice to them, they just take it for granted and don't feel any kind of basic human obligation to return even 10% the niceness and said nice guys then get tired of the uphill battle and decide that fucking other men might be a decent alternative.

Most females I know aren't interested in much besides having random sex with old men and guys whom you might believe are retarded upon meeting them. The "nice guy" thing is somewhat of a myth.
