Where I am the HIV rate is supposedly 2.2 percent and varies by region and city. I have met a large number of people living with hiv and most of them have been infected through heterosexual sex. I have also met a few who were infected through tainted blood transfusions. Others got it from their mother (there is a high probability of infection during birth if the mother is not on antiretrovirals, also a high probability if the mother is stupid and feeds the baby something other than breast milk, the milk carries the virus, other foods besides the milk open up wounds in a young babies stomach, infection occurs). To bring that 2.2 percent rate into perspective, out of fifty people at least 1 is infected. We did a girls leadership camp for 48 girls and they all were tested and one of them had hiv at age 13.

I'm not sure I like how the HIV studies are done in my country. Here, they just take every pregnant woman and tests them for HIV infection, that is where the 2.2 comes from. Every test I've seen has been the blood antibody or inside cheek antibody test. I was looking at some information about blood donor screening at a local hospital... all of it is tested and apparently 35 percent of the donors last year were HIV infected.

In terms of the probability of getting the disease, it depends on a whole host of factors. First of all, at what stage of infection is the person? If the person has been infected in the last 3-6 months (the period where antibodies against the virus have not yet been detected) the virus titer will be extremely high. The probability of the virus being in the vaginal fluid/semen is rather high then. Once the antibodies respond the virus titer goes down for a long time and the likelihood of HIV infection is decreased until AIDS starts developing. That is probably the time in which the chance is 1/1000. If you have sex with a full blown AIDS patient... I bet the risk would be much higher.

Other factors are also there. If there is any wound in the vag or on the penis of any kind the probability greatly increases, because its an open path to the blood stream instead of through a mucus membrane. So, if you are having sex in a country where the women put sand in their vaginas, your risk of HIV transfer is greatly increased. Also, if you have other STDs that create any sores in the genital region, definitely increased rate of transmission.

Also, women are much more at risk than men for infection. They have a much larger mucous membrane area, that is much more prone to other infections and tears that provide plenty of portals for HIV entry. Men are safer because the virus can only enter on the glans of the penis or under the foreskin if you are not circumsized. But, any sore/cut/chafing can increase that percentage especially just shoving in and pounding as hard as you can.

I think that you could probably drink as much HIV infected blood as you wanted as long as you don't have any wounds in your mouth, esophagus, stomach or small intestine, ulcer, chrons disease and extreme gingivitis sufferers would have a high chance of transmission.

In the case of the ridiculous HIV rates in South and East Africa. I don't know if it is a constant orgy. A friend of mine is a volunteer there teaching in elementary schools and a large number of the students are HIV orphans/are infected themselves. Mother to child transmission is huge in places that don't have proper antiretrovirals. Also, a lot of those countries still don't have proper screening on their blood, so anyone getting into an accident and getting a blood transfusion could be at high risk. Finally, there is the rather popular myth among men there that if you have sex with as many virgin girls as possible you will be cured of HIV. Because of the hymen breaking, the likelihood of transmission in those cases is huge.

HIV is no longer a death sentence. The drugs can keep you going for 10 20 30 years as long as you stick to the twice daily antiretroviral regime perfectly.

Basically, the exact risk of transmission having sex one time with an infected woman varies greatly, sometimes it can be extremely high under a certain set of conditions, or pretty low under other sets of conditions. I personally would rather wear the condom for awhile with a girl than take the risk of a lifelong disease.

Finally, the prostitutes in a nearby city charge 2 dollars equivalent for sex with a condom and 4 dollars equivalent for sex without a condom. They know its better for them to have it with the condom, though they're still stupid enough to take the extra money.
